Researchers report that screening for KRAS and BRAF mutations can reduce the cost of anti-EGFR treatment for metastatic colorectal cancer but with a very small reduction in overall survival according to a new study published on November 28 in the Journal of the National Cancer Institute.

Metastatic colorectal cancer patients whose tumors harbor mutations in KRAS (and to a lesser extent, in BRAF) are unlikely to respond to costly anti-EGFR therapies. Screening of patients who are candidates for these therapies for mutations in one of these genes (KRAS) has been recommended, with the goal of providing treatment to those who are likely to benefit from it while avoiding unnecessary costs and harm to those who are not likely to benefit. However, the real-world impact of mutation screening for both KRAS and BRAF is unclear.

To better understand the impact of mutation screening with regard to health outcomes, costs, and value, Ajay S. Behl, Ph.D., M.B.A., of the HealthPartners Research Foundation in Bloomington, Minnesota, and colleagues, performed a cost-effectiveness analysis that took into account the treatments, resection of metastases, and survival for the different types of metastases. They conducted patient-level decision analytic simulation modeling comparing four strategies involving KRAS and BRAF mutation testing to select treatments for metastatic colorectal cancer patients: no anti-EGFR therapy (best supportive care); anti-EGFR therapy without screening; screening for KRAS mutations only (before providing anti-EGFR therapy); and screening for KRAS and BRAF mutations (before providing anti-EGFR therapy).

The researchers found that compared with no anti-EGFR therapy, screening for both KRAS and BRAF mutations showed a very high (ie, unfavorable) incremental cost-effectiveness ratio, meaning it was very costly in relation to its benefits. Compared with anti-EGFR therapy without screening, screening for KRAS mutations saved approximately $7,500 per patient; adding BRAF mutation screening saved another $1023, with little reduction in expected survival.

The authors write, "In general, our results are less supportive of the use of anti-EGFR therapy than previous analyses, and they indicate lower cost savings from KRAS testing than previously reported. Although we cannot confirm that anti-EGFR therapy is a cost-effective use of health care resources, we can affirm that KRAS testing is cost-saving. BRAF testing may offer additional savings."

In an accompanying editorial, Josh J. Carlson, M.P.H., Ph.D., of the Department of Pharmacy, University of Washington, and Scott D. Ramsey, MD, PhD, of the Division of Public Health Sciences, Fred Hutchinson Cancer Research Center, both in Seattle, note two practical points highlighted by the study: that molecular testing is as much about generating cost savings by identifying nonresponders as it is about improving survival by identifying responders, and that good modeling must account for the fact that community practice (as opposed to clinical trials) "is messy." They write, "most importantly, this study of an unusually accurate test raises important issues that should be considered for other molecular tests in other settings."

YY.com soars on opening. Is the curse of the Chinese Internet IPO ...

Just a few months ago, the conventional wisdom among tech investors in both China and the US was that the IPO exit route was all but closed off for Chinese Internet companies. A series of accounting scandals and poor stock market showings ? by Dang Dang, VIPshop, Mecox Lane, Youku, and Tudou ? had besmirched the reputations of Chinese Internet companies just when they were starting to look unstoppable. Then Facebook?s disappointing IPO came along and made life harder for everyone in tech, not only the companies in China. Those factors delayed the public outings of companies such as AdChina, and, likely, ecommerce giant 360buy.

Last week, a previously low-profile company called YY.com struck what might come to be seen as the first blow in reversing the fortunes for Chinese Internet companies on the US public markets. YY is a communications and entertainment portal at which people can set up their own audio or video ?channels? to do anything from perform karaoke songs to discuss pig farming with peers.

After starting out as a Gamespot-like gaming news portal called Duowan, the company added impressive audio and video streaming technology that allows thousands of people to communicate clearly in a specific channel at any one time. The tech was initially used to help Chinese ?World of Warcraft? players coordinate their gaming. In 2008, the company added former WebEx engineer Tony Zhao as CTO and switched focus from portal to streaming. As of the middle of the year, it was hosting 11 million channels and 60 million monthly active users. The company was sitting on more than $100 million in cash, $95 million of which it had raised in venture backing from Morningside Ventures, Steamboat Ventures, GGV Capital, and Tiger Global Partners.

On November 21, YY went public on Nasdaq, and not only did the process go smoothly, but its stock has also registered healthy gains in the days since, climbing from the opening price of $10.50 per share to $12.65 (at time of writing). The IPO has also made YY angel investor and Chinese Internet superstar Lei Jun an even-richer man. He saw a hundred-fold return on the IPO.

YY is the first Chinese Internet company to take the plunge into a US stock exchange in eight months. As Quartz points out, the number of Chinese IPOs in the US has fallen from 41 in 2010, to 12 in 2011, to just two, including YY. VIPshop, which went public in March, raised 40 percent less than it sought, but in recent months it has experienced a comeback, climbing from $4.80 per share in late August to an all-time high of $13.11 today (at time of writing).

For Hany Nada, founding partner of GGV Capital, a major YY investor, many American perceptions of Chinese Internet companies of late have been unfair. There have been accounting frauds in Chinese companies ??including, prominently, at Sino-Forest ??but Nada says accounting fraud happens in US companies, too. ?There are bad apples in every basket,? Nada says. ?The question is ?How big is your basket???

Often the problems seen in Chinese companies that are hoping to go public, or have done so, are a reflection of inexperience and over-aggressiveness, Nada says. ?Many of the issues you hear about with Chinese companies is frankly just lack of experience rather than willful wrongdoing.? But the same kinds of things happen in the US, he points out ? just look at Groupon and Autonomy for two recent examples.

?It doesn?t get treated the same way as if there?s a Chinese company has a problem,? Nada says. If a Chinese company is seen to have problems, then other Chinese companies tend to get tarred with the same brush. ?I think they?re largely mistreated and treated unfairly just because of a few bad apples.?

However, strong companies always have the opportunity to go public, despite the market conditions, says Nada.

In YY?s case, there is certainly much to catch one?s interest. In July, I visited the company?s headquarters in Guangzhou, southern China. The company is housed in a five-storey building that?s tucked down a lane in a center-city complex once dedicated to light industry. The lobby has stone floors polished to look like marble, and an orange wooden sculpture that twists and builds on itself in a motionless fury like an unstoppable tornado.

About 1,000 people work at YY ? many of them wondering around in T-shirts and jeans, looking like they?ve stepped straight off a university campus ? but the company is still not that widely known even in China. Going public was one way to legitimize the company as a top place to work, YY?s CFO, Eric He, told me at the time. Top talent in China are often reluctant to work at startup or a company that does not have a high-profile reputation.

Until the IPO, few people realized how much money YY was bringing in. Perhaps that?s because for a long time it was making a loss. In 2011, it lost about $13.35 million, but this year it registered a profit of $8.9 million. A lot of its revenue had been coming from advertising and gaming, but in the last year the company has seen a surge in profits from its music division. It generates cash by selling virtual items that members use to reward singers they like in specific channels. Many of these singers are amateurs, but some are making tens of thousands of dollars a month just on YY.

Now, the company is building up its education monetization in a similar way. YY members can set up their own channels to teach lessons, on everything from math tutorials to English classes, for which students are willing to pay.

In the future, YY hopes to expand as a platform, playing host to an app economy in which third-party developers can build tools that improve the current offerings inside the channels. One example is virtual whiteboards for teachers, which could be sold cheaply, with YY taking a handsome cut, perhaps as high as 70 percent of each transaction ? a figure that Chinese companies can get away with.

There are no companies quite like YY in the US, although both Airtime and Google Hangouts use similar technology. In China, Tencent is trying to build a similar platform but so far doesn?t have a competitive consolidated offering. It does, however, have Weixin (the English-language version of which is WeChat). Now 200 million users strong, Weixin has a leg up over YY on mobile and satisfies some of the mass communication needs that YY serves. Another challenger is Changba, a mobile karaoke app that lets singers create and broadcast their own songs and accept virtual gifts from fans.

As the Internet emphasis in China increasingly shifts to mobile, YY will have to find ways to stem the momentum of both Weixin and Changba to keep that stock price ticking up.

Sophie Keller: 1 Awesome New Habit to Adopt for the Holidays

With the holidays upon us and it being a time when we get together with family and friends, I wanted to give you a way in which you can easily make other people's lives work and your own in the process. It is something that you might want to begin now and continue on a long-term basis, and that is to become a matchmaker.

When I say "become a matchmaker," I don't solely mean in the sense of connecting people up who would make good love matches, although that might be part of it. I am also talking about putting people together who have work, hobbies and other aspects of their lives in common. In this way, you go from being a matchmaker to becoming more of a connector. Indeed, you may be one already -- or you may know of one, two, or many, for that matter.

So how do you personally become a connector? Here are two ways:

1: You have to want other people's lives to work, regardless of whether yours is or isn't. By giving out that sort of energy, you will get rewards back in spades and you'll see, your life will automatically improve.

2: To become a connector, you cannot be the jealous type. You have to have the attitude that there is enough to go around for everyone. There is enough money, there is enough work, there are enough friends, there is enough love -- and by adopting that perspective, you will always have more than enough of all of the above.

So, with those two criteria in mind, think about whom in your life could benefit from connecting up with someone else in your life, whether it's a friend, family member, work associate or anyone you enjoy meeting at a holiday party. How could introducing these two people mutually benefit both of their lives, regardless of it being for love, work or friendship?

It's the holiday season and time for giving, and maybe that means it's time for you to be a connector, because one thing is for sure: Without connectors, we would all live very lonely lives.

Killing Them Softly | Review | U.S. Indie News, Filmmaker Interviews ...

Andrew Dominik Killing Them Softly Life imitates art, it could be argued, with Andrew Dominik?s latest film, Killing Them Softly, a talky genre effort with the repetitive din of the 2008 presidential elections buzzing in the background, and the economic recession nearly materializing as a physical character itself, so many times it is invoked as the impetus for each character?s action. Because of its heavily stylized and overt nihilism about the ignorant posturing of American life, Dominik?s latest film may not receive the championing reception it deserves, but several decades from now, a generation or two removed from the financial woes of 2008, should grant the film a reputation of note, even if the plot is perilously paper thin.

Johnny Amato (Vincent Curatola), who runs a dry cleaning business with underworld ties on the side, has come up with an idea to rob a mob-protected card game. In order to do this, Johnny hires Frankie (Scoot McNairy), a freshly paroled criminal, in need of some fast dough. For a second gunman, Frankie brings along a seedy Australian junkie, Russell (Ben Mendelsohn), currently operating his own dog theft business (not unlike a similar scheme by some fluffier characters in this year?s Seven Psychopaths). Because the card game?s dealer, Markie Trattam (Ray Liotta), knocked over his own card game once before, and not only got away with it but later bragged about it, Johnny Amato figures that if the game gets hit again, Trattam will take the fall, even if he?s found to be completely innocent. After Frankie and Russell commit the deed, the mob brings in a seasoned enforcer, Jackie Cogan (Brad Pitt), who works for the infamous Dillon (Sam Shepard), a crotchety type currently suffering from debilitating health issues. Because Jackie personally knows one of the responsible parties, he contracts Mickey (James Gandolfini), (much to the chagrin of the Driver (Richard Jenkins), who must seek approval for all of Cogan?s budgetary issues from the people in charge), to help complete the job.

Dominik cites Peter Yates? underrated 1973 Robert Mitchum starrer The Friends of Eddie Coyle as the originating inspiration, motivating him to explore other novels by George V. Higgins, who wrote that source novel, which lead him to stumble on Higgins? Cogan?s Trade. Dominik?s modernization of Higgins? novel, however, tends to makes every obvious 2008 reference seem anachronistic, creating a sort of ragged resistance for the unsubtle modern day woes to tether into the brassy underworld landscape. Just as Yates? film opened to less than satisfactory box office results, Killing Them Softly has weathered critical lashings since its Cannes premiere earlier this year, despite headlining aging heartthrob Brad Pitt, reuniting with Dominick after their 2007 critical success The Assassination of Jesse James by the Coward Robert Ford.

Dominick?s crime flick is more of an anti-thriller, instead taking on the capacity of a mean-spirited character study. But whereas Mitchum was our titular protagonist of Yates?, here, we?re given a stained composite of America, standing in as a never seen femme fatale, an uber bitch siren that leads men into her sanctum where they struggle in futility, until they expire, like moths singeing themselves on the cruel glass of the heated lightbulb. We certainly never get beyond the surface of Pitt?s not completely uncaring Jackie Cogan, who prefers to kill his targets softly, from a distance, so as not to be tainted by the emotion of it all. His fear of emotion, in fact, gives us the highlight of the second half of the film, where we get a down and out James Gandolfini as a has-been East coast hit man, flown in to kill one of the targets that Pitt knows personally. But even as the first half of the film, which engenders genre rules, giving us an intense heist sequence which ripples into small bursts of excessive and beautifully photographed violence, Killing Them Softly pulls a bait and switch, and turns into an arduous exercise spinning endlessly around the notion of money.

Just as Dominick bludgeons us with the on point snippets of sound bites from McCain and Obama, so does he carefully choose his modulated soundtrack filled with vintage tunes that not only help make the film feel like a throwback but only further highlights the false veneer of entertainment as well as the lip service of the political machine, neither managing to address real life, but only helping us escape from it into notions, ideas, or distracting performances.

China praises engineer in aircraft carrier program

BEIJING (AP) ? China's government gave hero's praise to a senior engineer on its aircraft carrier program who died of a heart attack after witnessing the first landing of a plane on the ship, underscoring the project's huge national prestige.

State broadcaster CCTV ran news of Luo Yang's death as its first item on Monday's noon news broadcast, a high profile for a scientist who was previously unknown outside of the carrier program. Luo, 51, oversaw the development of the J-15 fighter-bomber planes designated for the ship, which is called Liaoning after the province where it is based.

The coverage illustrates the priority Beijing has placed on the program, seen as representing China's rise from poverty to economic and political might over the past three decades. The carrier was built in the former Soviet Union and is seen as a test platform for future Chinese-built vessels.

Few details were available about Luo, who died Sunday. A man who answered the phone at his employer, the Shenyang Aircraft Corp., confirmed his death but declined to give details. The company produces the bulk of China's modern military jets ? many of them, like the J-15, derived from Russian models.

The official China Daily newspaper said Luo had felt unwell while on board the ship but continued to monitor landings. He suffered a massive coronary after returning to his hotel and died on the way to hospital, the report said said.

Chinese academics assigned to signature government projects such as the carrier are often under enormous pressure, and stress is considered among the biggest killers of the country's intellectuals. China overcame major technical hurdles to make the carrier seaworthy after buying the half-completed hulk from the Ukraine and towing it to China, minus its engines, weaponry and navigation systems.

Most of that work was carried out in the northeastern port of Dalian, to which the carrier returned on Sunday following its latest round of sea trials.

Despite the breakthrough in air operations, the ship is still years away from being battle-worthy. China's navy still needs to show it can operate large numbers of planes at the same time and organize a carrier battle group that includes submarines and support ships.

"The first landing may be a milestone, but it's just the beginning," said Toshi Yoshihara, professor of Asia-Pacific studies at the U.S. Naval War College in Rhode Island. "How the Chinese will handle aircraft losses, which are inevitable, is a better indicator of Beijing's determination to become a carrier."

The J-15 is a Chinese adaptation of the naval version of Russia's Su-33.

In Washington, the State Department said the U.S. was continuing to monitor Chinese military developments carefully.

Spokeswoman Victoria Nuland urged Beijing to be as transparent as possible about its military capabilities and intentions and to use its capabilities, including the aircraft carrier, in a way conducive to supporting peace and stability in the Asia-Pacific region.

Freed Russian scientist: 'Nothing has changed'

Gray, pale and thin, Valentin Danilov has changed more than the country that jailed him in 2004 for selling state secrets to China.

The 66-year-old Russian physicist, whose face is now criss-crossed with deep wrinkles, could not be blamed for suffering from "deja vu" when he was released on Saturday from a Siberian penal colony on spying charges he says were politically motivated.

President Vladimir Putin, now 60, is back in the Kremlin for a third term, corruption is rife, the unreformed economy is creaking under the weight of its dependence on energy exports, and opponents are still being imprisoned.

Danilov, whose case human rights activists say is evidence that Putin uses Russia's weak courts to persecute his enemies, sees little hope of rapid change.

"Nothing has changed," Danilov said in an interview, putting some of the blame on Russia's 143 million people.

"The authorities do not descend on us from the moon. They are the choice of the nation. So the authorities reflect the state of the nation," he told Reuters a few hours after his release from the high-fenced penal colony.

News of one major change did reach him during his last year in the colony in a grimy industrial area outside the Siberian city of Krasnoyarsk, 4,000 miles east of Moscow - reports that people had taken to the streets to protest.

Demonstrations against Putin in Moscow and other big cities began a year ago, caused by anger over allegations of fraud in a parliamentary election won by the Kremlin leader's party, but they have largely lost momentum and the opposition is divided.

Reflecting on the possibility of free and fair elections, and the possibility of political upheaval, Danilov said: "The nation is not yet ready."

New crackdown?
Dressed formally in a red tie and grey jacket, Danilov was speaking in an apartment in the city where he was born and jailed, and which was once part of Soviet dictator Josef Stalin's network of Gulag prison camps.

A researcher at Krasnoyarsk State University, he was first arrested in 2001. He admitted selling information about satellite technology to a Chinese company but he, other scientists and human rights activists said the information had already been available from public sources.

An initial decision to acquit him was overturned, and he was sentenced to 14 years in prison in a second trial. A Krasnoyarsk court granted him parole earlier this month, citing good behavior and poor health.

Asked how he felt about finally stepping outside the prison walls, he said "there were no feelings," but added that he had no regrets and that he regarded himself as a political prisoner.

At the time of Danilov's trial, Putin's opponents said the president was clamping down on academics who had contacts with foreign countries. They say his release showed that the Kremlin no longer regarded the physicist as a threat.

Opposition members see similarities between what happened to Danilov and the pressure being put on them now in Moscow.

Citing legal cases such as the sentencing of members of the Pussy Riot punk band over an anti-Putin protest in a Russian Orthodox Church, they say the Kremlin is using the legal system to smother dissent.

Putin denies this but several opposition leaders face criminal charges and the parliament has adopted a slew of laws over the last half year which opponents say could be applied against them.

These include tightening checks on lobby and campaign groups that have foreign funding, forcing them to register as "foreign agents," and broadening the definition of treason.

"As for President Putin, I guess everybody would be the same as him in his place. The court makes the tsar," Danilov said, avoiding direct criticism of the president but condemning the circle around him.

"The problem is not one of law but of how the judging is done."

He read widely about Russia's legal system during his time in prison, and said the judiciary was still open to political manipulation.

No plans to enter politics
After nearly a decade behind bars, including in colonies populated by murderers, Danilov's brown eyes are still penetrating and his wits sharp. He deflects questions about his health but is not a broken man.

He does not want to look back, refusing to go into detail about his life in prison or his health.

"It's like serving in the army, only that a man in the army has fewer rights. By taking the military oath, a soldier gives up some of his rights. While in prison, the prisoner can at least call in a lawyer and make complaints about abuse of rights," he said.

Among people he admires, he listed several Putin critics - opposition leader Boris Nemtsov, former world chess champion Garry Kasparov and human rights campaigner Lyudmila Alexeyeva.

He praised the entry into politics of Mikhail Prokhorov, a rich tycoon who challenged Putin in the March presidential election while denying accusations of being a "Kremlin stooge."

Beyond retirement age, worn down by his years in prison, Danilov signals that starting a new fight with the state or taking on a role in opposition is the last thing on his mind.

He wants to return to work to try out ideas he developed while he had time on his hands in jail. He also says he is ready to play an advisory role on how to reform Russia's outdated penal system.

He aims to rebuild his strength and family ties with his daughter, granddaughter and wife of 41 years who lives in Novosibirsk, also in Siberia.

Danilov said he had no plans to flee Russia or deal with space research again. He plans to keep in touch with people he met behind bars, including a man sentenced for murder whom he helped to obtain higher education.

Putting a positive spin on his years in jail, he said: "They say that to get to know a country well, one must visit its cemeteries and prison. I used to visit cemeteries often and now I've been to prison too.

"So you can really believe me when I say I know perfectly fine now what Russia is," he said.

He paused for a moment and, smiling, switched to English to quote the title of a Shakespeare play: "All's well that ends well."

Local real estate agent selected president-elect of national trade group

Steve Brown, co-owner of local real estate firm Irongate Inc. Realtors, has been named the National Association of Realtors 2013 president-elect.

Brown was installed Nov. 12 in the officer position of the national industry trade group, Irongate said. This continues his track to become president of the group in 2014. This year, Brown was first vice president of National Association of Realtors.

Brown has been a real estate agent for more than 30 years. Recently he was recognized by Ohio Association of Realtors with the 2012 Vince Aveni Award for Lifetime Achievement. He was president of the Dayton Area Board of Realtors in 1995, and of the Ohio Association in 2002, according to Irongate.

Irongate has six offices in the Dayton area and more than 275 real estate agents.

'The X Factor' Results: Arin Ray And Beatrice Miller Sent Home In Double Elimination (VIDEO)

Even a holiday as cheerful as Thanksgiving couldn't stop Simon Cowell from breaking a few hearts on "The X Factor" results show this week.

After a particularly emotional night on Wednesday, which saw the contestants dedicating songs to the people who inspire them most, Thursday saw two acts culled from the competition. The first came as little surprise, with 17-year-old Arin Ray earning the lowest number of votes after a lackluster performance of "Hero."

The other two acts with the lowest scores were CeCe Frey (who sang "Wind Beneath My Wings" on Wednesday) and Beatrice Miller ("Chasing Cars"). The pair left everything on stage with their "save me" songs -- CeCe performed "Because Of You," while Beatrice tackled "White Flag," and both struggled to finish the renditions because their emotions were running so high.

It was a rough night for coach Britney Spears; after she lost Arin, the other three judges chose to eliminate Beatrice too. Simon Cowell told Beatrice that he thought the competition was "all getting too much for you ... I don't think this is the right time for you" while the 13-year-old sobbed beside CeCe. Despite losing two contestants, it wasn't all bad news for Britney's team: Her other 13-year-old prodigy, Carly Rose Sonenclar, claimed the top spot on the leaderboard after a heartfelt and confident performance of "Somewhere Over the Rainbow" on Wednesday night.

It?s been a long while since William Caxton set up his first printing press in Westminster and provided a mechanism to help spread the written word that would eventually strike at the heart of Reformation England. Caxton?s invention in 1483 allowed for the mass production of printed matter which, in time challenged the authority of the established church, lead to the publication of?the King James Bible, and brought about the Age of Enlightenment and the Industrial Revolution.

Today, we are standing on the shoulders of this great giant.?As Caxton transformed important words into powerful messages through innovation in technology over 500 years ago, so we now must learn to do similar with big data ? that?collection of?data sets?so large and complex that it has become difficult to process using on-hand database management tools -?in the 21st century.

But, is the time right?

We?ve now had around 20 years of the commercial Internet with organisations embracing innovation to find new markets globally, provide a better service to their customers, simplify processes, and generally make life easier. The explosion of social media has made our personal and business relationships more interactive online, but we are faced with the enormity of information overkill that makes any informed decisions difficult.

How do we make sense of big data,?prioristise what?s important, value and secure it?

Start-ups are among those best placed to make the most of new technology, but they may be stopped in their tracks by more established players who will protect their own interests. Many small businesses will find it difficult to defend legal challenges to intellectual property rights or stop large corporate organisations from using IP laws to restrict consumer behaviour in particular markets.

Dealing with big data brings a whole plethora of legal problems such as intellectual property rights; interoperability; licensing; platforms and processing, and services out. Who is responsible for big data, is it accurate and will algorithms that drive the data be accessible or remain commercially sensitive? Can we anonymise data so that it doesn?t breach privacy and data protection rules?

A 2011?McKinsey?report?suggests suitable technologies include?A/B testing,?association rule learning,?classification, luster analysis,?crowd sourcing,data fusion?and?integration,?ensemble learning,?genetic algorithms,?machine learning, natural language processing,?neural networks,pattern recognition,?anomaly detection,?predictive modelling,?regression,?sentiment analysis,?signal processing,?supervised?and unsupervised learning,?simulation,?time series analysis?and?visualisation.

Additional technologies being applied to big data include massively parallel-processing (MPP) databases,?search-based applications, data-mining grids, distributed file systems, distributed databases, cloud computing platforms, the Internet, and scalable storage systems.

But whilst big data may be difficult to work with using relational databases and desktop statistics and visualization packages, requiring instead massively parallel software running on tens, hundreds, or even thousands of servers, let?s not forget the end game. Everything organisations do, irrespective of size or sector must be viable financially and serve the bottom line. Solutions therefore need to be scalable and reflect the value in the big data being collected.

Caxton?s simple printing press changed the world. We?ve just got to make it a smarter planet.
